In addition, a copy of the referring physician’s signed order is required and can be faxed or sent via secure email.

A Care Representative will work on the Member’s behalf to:

  1. Locate a provider in the Member’s requested area
  2. Confirm pricing to Member
  3. Collect payment from Member
  4. Provide financing options, if applicable
  5. Assist with appointment scheduling
